According to Fabrizio Romano, Betis will pay an initial £19m plus £2.6m in add-ons to secure the transfer.
United signed Antony from Ajax in 2022 for a significant fee, but he has failed to meet expectations at Old Trafford.
The winger managed just 12 goals and five assists in 96 appearances before losing his place under Ruben Amorim.
Betis Bring Back A Proven Performer
Antony rediscovered his form while on loan with Real Betis, where he produced 14 goal contributions in only 26 matches.
That spell convinced the Spanish club to bring him back permanently, with a medical scheduled in Seville this week.
The deal also includes a 50% sell-on clause, meaning United could profit if Betis sell him in the future, reports
For Betis, the return of Antony represents a statement signing as they look to challenge for European qualification again.
Manchester United, meanwhile, continue to reshape their squad, focusing on adding depth while moving on players surplus to Amorim’s plans.
If the deal is finalised, Antony’s exit will mark one of United’s most significant deadline-day sales in recent years.
